Here you will find a wide range of information around end of life care whether you are a patient, a relative, carer, health or social care professional or just interested in learning more about end of life care and planning for the future.
It is difficult to exaggerate the importance of good end of life care. After all, we only ever have one chance to get it right.
Over 500,000 people die each year in England of whom two thirds will be over the age of 75.
The large majority of deaths follow a period of chronic illness such as heart disease, cancer, stroke, chronic respiratory disease, neurological disease or dementia and as such could be predicted. If death can be predicted, it can be prepared for so that everyone in their last year of life can receive the best quality end of life care regardless of their diagnosis or where they live.
This is the aim of the National End of Life Care Strategy.
This Network is working hard to implement the strategy and you will find information about what we’re doing on this website.
