The Knowledge, Skills and Confidence Audit has been designed so that data can be collected from the care home representative around their abilities across sixteen key areas of End of Life Care.  This audit should assist in preparing feedback for the care home representative and in developing reports to evaluate the implementation of the Six Steps to Success programme.

There are several audit tools which will support collation and analysis of the data.

Knowledge, Skills and Confidence Analysis Audit Tool Guidance 

This document provides an overview of the Knowledge, Skills and Confidence Audit  which is undertaken by each care home representative at the beginning and the end of the programme.  The document also contains instructions on how to use the Analysis Audit Tool which has been developed to support the collection and analysis of the audit results. 

 

 

Knowledge, Skills and Confidence Paper Audit -Pre-programme 

This paper audit tool will allow you to collect information on the knowledge, skills and confidence of the care home representative at the beginning of the programme.  The paper version can be printed for care home staff to complete by hand if there is limited access to computers.  Once complete the data collected in this audit can be transferred into the electronic Knowledge, Skills and Confidence Audit Analysis Tool.

Knowledge, Skills and Confidence Paper Audit - Post-programme 

This paper audit tool will allow you to collect information on the knowledge, skills and confidence of the care home representative at the end of the programme.  The paper version can be printed for care home staff to complete by hand if there is limited access to computers.  Once complete the data collected in this audit can be combined with data from the Pre-programme audit in the electronic Knowledge, Skills and Confidence Audit Analysis Tool.

This paper audit tool will allow you to monitor the care home staff in their knowledge, skills and confidence across sixtreen key areas of End of Life care.

Knowledge, Skills and Confidence Audit Analysis Tool 

This spread sheet allows data entry of the information collected from the Pre-Programme and Post- Programme Knowledge, Skills and Confidence Audit (Paper Copy).  Once the data is entered the tool will analyse and produce graphs for feedback.  Information about the use of this tool is can be found in the Knowledge, Skills and Confidence Analysis Audit Tool Guidance. 

 

 

Knowledge, Skills and Confidence Audit Cohort Tool Guidance 

This document provides guidance on the use of the second Knowledge, Skills and Confidence tool. This Cohort Tool has been developed to work alongside the individual care home representative audit tool and allows the quick analysis of a cohort of care homes to give an indication of how the cohort has developed over the course of the programme. 

 

 

Knowledge, Skills and Confidence Audit Cohort Tool 

This spread sheet will allow you to use data collected from multiple care home representatives in the Knowledge Skills and Confidence Analysis Tool to provide feedback on a cohort of care home representatives.  This tool will use averages to analyse the knowledge, skills and confidence of cohorts of care home representatives and details and instructions on the use of this tool can be found in this document.
