Aim: To commence the Six Steps to Success programme 

 

Objectives:


By the end of the session the Care Home Representative will be able to:

• Identify the National, Regional and Local end of life care drivers

• Understand the programme

• Commence the audit process

• Have an understanding of their role and responsibilities

• Commence an End of Life Care Policy.

It is recommended that Care Home Managers are invited to Induction Workshops alongside the Care Home Representative to gain an understanding of the programme content and the support that will be required to implement change.
