Throughout the programme the care homes are asked to collect information on the deaths of their residents.  This will help to develop the Six Step to Success Programme and it is important to capture data to demonstrate the effectiveness of the programme and identify the provision of end of life care in relation to nationally recommended end of life care tools.  Several tools have been developed to support the collection and anlalysis of this information.

Post Death Information Audit Tool Guidance 

This document provides guidance on the completion of the Post Death Information Audit and provides instructions on how to use the Post Death Information Audit Tool to provide analysis of the information collected from each care home. 

 

 

Post Death Information Pre-Programme Audit 

Care homes are asked to collect information on the deaths of residents that occured in the six months prior to joining the programme.  This document can be used to collect this information and can be completed as care home representatives look back over their records.

Post Death Information Ongoing Audit 

Care homes are asked to collect information on the deaths of residents that occur while they are undertaking the programme.  This data collection form can be completed each time a resident dies in order to gather vital information which can help identify any developments and challenges in the provision of end of life care.

Post Death Information Audit Tool 

This audit tool has been developed to allow the comparison of pre-programme deaths and ongoing-programme deaths.  This analysis will help to highlight areas where the care home has made improvement in the provision of End of Life care and also areas where the home may be facing challenges.  Guidance on how to use this tool can be found in the Post Death Information Audit Tool Guidance. 

 

 

Post Death Information Post-Programme Audit 

Care homes are encouraged to continue collecting data on the deaths of residents after they have completed the programme.  This will help the care home monitor their provision of End of Life care and will promote continued organisational development.  This form can be used to collect this information.
